Clinical and service context
- ADHD recognition and care: NICE NG87 recommendations, especially continuity of care. National guidance; it does not decide an individual prescription or determine fault in a case.
- System pathway concerns: NHS England, independent ADHD Taskforce report (2025; page updated 2026). A system review and recommendations, not evidence that a particular local action was implemented.
- Safe care framework: CQC guidance on Regulation 12. A provider standard, not an individual regulatory finding.
- Disability threshold: Equality Act 2010, section 6. The statutory definition depends on substantial, long-term effect; a diagnosis alone does not decide every adjustment.
Historical local policy copies
- CWPT Complaints Policy v9.1 — supplied historical copy; check the trust's current route and deadlines.
- CWPT Safeguarding Adults Policy v5.0 — institutional policy, not a decision about any individual's threshold.
- CWPT Freedom to Speak Up Policy v6.5 — staff route, distinct from patient complaints.
- Coventry and Warwickshire ICB ADHD under-25 policy v3 — historical commissioning copy with an age scope; do not apply it to adult care or assume it remains current.
First-person record and permitted commentary
Journey and Accountability report a first-person chronology. Three original video files now accompany the accountability page. A clip records the speaker and date; it does not independently establish a clinical diagnosis, causation or the full institutional record.
- Professor Asherson, 24 August 2026 and 27 August extract and permission. His comments have the limits stated on the accountability page: he was not involved in care and did not have the full clinical details.
- Professor Shakespeare, 1 September 2026, extract and permission. A view on access and accommodation, not a judicial determination.
Reading the pages
Where an individual page reports a letter or meeting but the underlying personal record is not published, read it as the advocate's account. Private clinical letters and complaint submissions are not exposed here. How the source method works explains the distinction between source, claim, inference and proposed action.
