1. Keep the source: record the issuer, title, version and date. Preserve an original when permission and privacy allow.
  2. State one claim at a time: attach the exact source and distinguish its words from a paraphrase.
  3. Mark the leap: separate documented fact, first-person account, inference, hypothesis and requested remedy.
  4. Keep the counterpoint: if a response disputes the claim, record it with its date and the specific issue it addresses.
  5. Check implementation: a response, review, assurance or promise is not a completed treatment, published pathway or closed action.
  6. Correct visibly: a corrected date, scope or attribution should identify what changed and why.

The title is a project name: it is not an ISO, BSI, statutory, regulatory or professionally endorsed standard. It does not certify this website or any case finding.

Privacy boundary

The public archive carries policy copies, permitted extracts, the project method and the first-person video already presented on the site. It does not carry the personal clinical correspondence, submissions or working contact lists. Removing a postal address and NHS number alone would not make a clinical record anonymous.
