Who is responsible?

Separate the next care decision from complaints, regulation and investigation.

1 · Start with ownership

A complaint acknowledgment does not itself produce care. Ask which clinician or service owns the next decision, what the decision is, when it will happen and what protects you while you wait.

England’s local service complaints usually belong with the provider or ICB; NHS England has a defined remit, rather than responsibility for every complaint.

Keep two requests visible: the care you need now, and the investigation of what went wrong. Ask for a named owner and deadline for each.
